Employment Verification & Tax Forms
For Verifications of Employment: Former employees must first contact “The Work Number” service at 1-800-996-7566, give them Zapata’s code (can be obtained from Zapata HR) and obtain the key to give the verifier. This key will give the verifier permission to receive the employee’s information. The employee should confirm with “The Work Number” service, the exact information they want shared before giving their key to anyone. For verifiers, “The Work Number” service can be contacted at 1-800-367-5690 or by visiting www.theworknumber.com, or by emailing info@theworknumber.com.
Requesting 1095-C Forms: Zapata Technology does not automatically furnish 1095-C forms. Any employee wishing to receive a copy of their 1095-C form for the calendar year can request a copy from Zapata Technology Human Resources the following year as early as January. (So, you can request 2025’s form in January of 2026) To make the request, simply submit the contact form above and select “Employment Verification & Tax Forms (HR)”, or mail your request to the address below:
